The County of Los Angeles would like to photograph the participant receiving services through the Economic Mobility Initiative (EMI) run by the Department of Economic Opportunity (DEO) and receive the right to use and publish photographs and/or video images of the participant for uses including but not limited to, any and all advertising, promotion, and marketing related to the EMI.

The participant has the right to give or withhold their permission to allow the County to photograph or videotape the participant and the decision on whether to permit the County to photograph or videotape the participant will not affect the ability to receive social service benefits through the EMI.

By giving permission under this agreement, the participant acknowledges that the County may photograph, videotape, and release the participant’s identity use in the DEO intranet, the DEO public website, a County Newsletter or other publication promoting County services and programs. The participant will receive no money or other benefits from the County of Los Angeles or any other party as a result of consenting to the release of such information and is not required to provide an interview or release any information to the media for this use.

Before agreeing or disagreeing to this photo consent and release agreement, the participant acknowledges the careful review and full understanding of its terms. If approved, this photo release will stand for one year from the date of receipt.

The County of Los Angeles would like to collect stories that capture the experience the participant has while receiving services through the Economic Mobility Initiative (EMI) run by the Department of Economic Opportunity (DEO) and receive the right to collect content including, but not limited to, short and long form quotes, testimonials, feedback, and/or context on the participant’s achievements, previous background, needs as related to the program. The participant may share this information through channels including, but not limited to, in-person and/or virtual interviews, phone calls, or emails.

The participant has the right to give or withhold their permission to allow the County to request stories about the participant’s experience and the decision on whether to permit the County to do so will not affect the ability to receive social service benefits through the EMI.

The participant retains the right to decline any story request. The participant also retains the right to a preferred identifier related to personal name (e.g., full name, first name, initials, anonymous) and business name (e.g., full name, location, anonymous), to be shared per story request.

By giving permission under this agreement, the participant acknowledges that they are willing to share stories and that the County may release any shared information in the DEO intranet, the DEO public website, a County Newsletter or other publication promoting County services and programs.

Before agreeing or disagreeing to this story consent and release agreement, the participant acknowledges the careful review and full understanding of its terms. If approved, this story release will stand for one year from the date of receipt.
